Brian Holzer, MD, MBA

Brian-Holzer_0222.pngBrian Holzer, MD, MBA, is president of HM Home and Community Services (HM HCS), a payer and provider agnostic company launched in 2016 within Highmark Health, a health and wellness organization and the third-largest integrated healthcare delivery and financing network in the nation. The vision of HM HCS is to transform quality, service and value in the post-acute care space through an innovative contracting and care management model. HM HCS is incorporating an innovative strategy aimed at shifting post-acute care contracts, starting with skilled nursing facilities and home healthcare agencies, from a fee-for-service incentive model, to a quality and outcomes-based incentive model.  By working with at-risk payers and providers to leverage their claims and utilization management data, HM HCS will align pay-for-performance incentives to quality and outcomes. 

Dr. Holzer joined Highmark Health in 2013, initially serving as senior vice-president for Diversified Services at Allegheny Health Network (AHN). Dr. Holzer launched and continues to lead a western Pennsylvania home- and community-based "Healthcare@Home" model serving AHN and non-AHN acute care hospital systems, physician practices, and post-acute care healthcare organizations such as skilled nursing facilities. The Healthcare@Home program leverages a team of transitional care coordinators and a portfolio of integrated companies (Home Health, Infusion, Medical Equipment, Palliative Care and Hospice) to provide differentiated quality, service and value for patients and key stakeholders for post-acute and chronic disease care delivered in the home setting.

Dr. Holzer’s professional and educational healthcare experiences include an MD from Drexel School of Medicine with a one-year general residency program in the University of Pennsylvania Health System. Dr. Holzer went on to earn an MBA with a concentration in healthcare management from The Wharton School of Business and has held healthcare consulting and various strategy, operations, and sales and marketing roles in the biotechnology industry.

Dr. Holzer currently serves as the board chair for AHN Home Infusion, AHN Home Medical Equipment and AHN Home Health and Hospice.  He is also active in the Pittsburgh community with the JDRF and the American Cancer Society and is a member of the board of directors of The Pittsburgh Regional Health Initiative and The Children’s Institute.
